  • The form covers BC, Alberta, Ontario and Canadian federal corporations.
  • The form is a fillable PDF, so you can enter the information directly onto the form.
  • However, you won't be able to save the information on the form (unless you have a full copy of Adobe Acrobat), so be sure to print it out before closing it.
  • Fill it out as best you can. We'll cover any questions you have after we receive the form.
  • Once printed, please fax the form to us at 604-272-6959 or scan and e-mail it to us.
  • We'll respond to you once we receive it. Give us a day or two. Check back if you don't hear from us.
  • The Law Society prefers that we say this: None of downloading the form or sending it to us creates a lawyer-client relationship with you. Don't use the form to send confidential information.
